What the ON00646 is for

The Mandatory Special Necessities benefit helps ODSP recipients and their family members with costs no other provincial, federal or private program covers. It funds travel to medical treatment, to local alcohol and drug recovery groups, and to mental health therapy prescribed by a physician or psychologist. It also funds diabetic supplies, surgical supplies and dressings, and items and devices needed because of a surgical, radiological or medical procedure or a disease. Approval is based on the most economical cost.

The form is for people already on ODSP, so the applicant needs their member ID. Applying for ODSP itself is a separate process.

Who completes it

The applicant fills in their name, date of birth and member ID, and signs the declaration and consent. Without the signature the application isn’t approved. For a child under 16, someone lawfully authorized signs. A health care professional licensed in Ontario completes only the sections that match the patient’s documented needs, then signs and gives their registration details.

Patients usually bring the printed form to an appointment with page one filled in, or fill it in at the front desk, and the clinic completes the remaining pages by hand. In Medulla, you send the applicant information and declaration to the patient over a secure link. They fill it in on the form from any device, and their answers are on the PDF where you complete the clinical sections, so it goes back as one complete application.

How to fill in the health care professional sections

Medical Transportation

Say whether a medical condition prevents the patient from using public transit or para-transit where it’s available, and whether they need specialized patient transportation. Then list each destination for the next 12 months: the provider or facility, number of visits, the months the travel starts and ends, and whether an attendant or overnight stay is needed.

Wound care, lymphedema, incontinence and diabetic supplies

For each category, answer whether the patient has a condition that needs those supplies, and choose a duration of 3, 6 or 12 months or indefinite. ODSP staff use the duration you choose. For gestational diabetes, give the expected delivery date.

Aids, prosthetics, braces, respiratory and monitoring devices

For self-care aids, limb prosthetic items, braces and splints, respiratory devices such as positive airway pressure supplies, and monitoring devices, answer yes or no, give the duration, and name the specific item. Attach assessments and vendor quotes if you have them.

Signature and Information

Sign, then complete the statement with your profession, regulatory college, registration number and office contact details.

Common reasons it’s delayed

  • The applicant’s declaration unsigned.
  • A psychologist completing sections beyond Medical Transportation.
  • Transportation listed without visit counts or dates.
  • No duration chosen, so the caseworker can’t set an approval period.
  • No vendor quotes for devices.

Submitting and who pays

The patient submits the form online through MyBenefits, at the local ODSP office, or by mail to the Markham address on the form. Physicians bill OHIP using the fee code printed on the form. Other health care professionals invoice ODSP for $25, with their details, the applicant’s name and member ID, sent to the same Markham address.

Questions

Questions we hear a lot.

Can a psychologist complete the MSN form?

Only the Medical Transportation section. The surgical supplies, items and devices sections must be completed by a physician, nurse practitioner, registered nurse, physician assistant, occupational therapist or physiotherapist.

How is the physician paid for the MSN form?

Physicians bill OHIP using the fee code printed on the form. Nurse practitioners, registered nurses, physician assistants, occupational therapists, physiotherapists and psychologists send ODSP an invoice for $25.

Is there a minimum for medical travel?

Yes. ODSP's directive says travel costs are paid when they reach $15 or more per benefit unit in a month.

How does the patient submit it?

Is patient information secure?

Yes. Medulla has a SOC 2 Type II report from an independent auditor, is HIPAA, PIPEDA, and PHIPA compliant, and is built and hosted in Canada, so patient data stays in the country and never gets pasted into consumer AI tools. Everything is encrypted in transit and at rest. See how we protect patient data.
