AI Plan Reminders for Canadian primary care clinicians

Catch what the plan missed before the patient leaves.

A clinical assistant that has your back on every visit. Once the note is ready, Medulla reads your plan against what was said in the room and what is on the chart. A red flag with no safety-netting, or a new prescription that clashes with last month's labs, shows up right under the plan while you can still act on it, so nothing important slips past.

Plan and next steps

  • Naproxen 500 mg twice daily for knee pain
  • Physio referral
  • Review in 4 weeks

NSAID planned with reduced kidney function and an ACE inhibitor

Her last eGFR was 48 and she takes lisinopril. Naproxen raises the risk of acute kidney injury and hyperkalemia.

Chart: eGFR 48 (Aug), lisinopril 20 mg. Visit: "The knee has been bad for weeks."

The difference

A second read of the plan, while the patient is still there.

You hold a whole chart in your head for every patient, visit after visit, all day. On the busiest days, something can slip. Medulla reads every plan against the patient you are seeing, so you are not carrying that alone.

The bolted-on way

  • The scribe writes the plan down as dictated. Cross-referencing is left to you.
  • A missed safety-net or a drug and lab clash surfaces later, in a callback or a chart review.
  • Decision support lives in another tab or a reference app you have to remember to open.
  • Generic EMR alerts fire on everything, so you learn to click past them.

The Medulla way

  • Every plan is checked against the conversation and the patient's chart as soon as the note is ready.
  • Gaps are flagged in the note, under the plan, while the patient is still in the room.
  • Each reminder shows its evidence: what the patient said, what the chart shows, and a link to the reference.
  • Rate any reminder with a thumbs up or down, right where it appears.

Reminders suggest, you decide. Nothing in the note or the chart changes unless you change it.

How it works

Flagged in the note, at the point of care.

  1. 01

    Scribe the visit as usual

Medulla writes the note during the visit, the same as always. There is nothing extra to open or run.

  2. 02

The plan gets a second read

Once the note is ready, Medulla checks the plan against what the patient told you and what is on their chart: allergies, medications, and recent results.

  3. 03

    Act before they leave

A gap shows up as a reminder right under the plan, with the evidence behind it. Add the safety-netting, change the prescription, or carry on.

Reminders

Every reminder shows where it came from.

  • Grounded in this patient

Reminders come from the visit and the chart, so a flag about kidney function cites this patient's eGFR and medication list.

  • Evidence you can check in seconds

Each reminder quotes what was said in the room and what the chart shows, so you can judge it at a glance.

  • A link to the reference

Every reminder links to the clinical reference behind it, one click away when you want the detail.

  • Right under the plan

Reminders sit in the note on the encounter page, in the Medulla drawer. No second tab, no separate decision-support app.

Questions

Questions we hear a lot.

What does a plan reminder flag?

Crucial misses only. A red flag with no escalation or safety-netting, a prescription that clashes with an allergy, a current medication, or recent labs, or an abnormal result that nobody addressed.

Where does it get its information?

From the visit itself and the patient's chart in your EMR, the same patient context AI Insight and Ask use. There is nothing to upload or paste.

Does a reminder change my note?

No. A reminder is a suggestion shown under the plan. You decide whether to act on it, and the note only changes if you edit it.

Will it bury me in alerts?

No. Reminders are kept for crucial misses, so most visits have none. Routine screening, lifestyle advice, and judgement calls are left to you. Rate any reminder with a thumbs up or down to tell us whether it helped.

Do I have to leave the encounter page?

No. Reminders appear in the note, in the Medulla drawer beside your encounter. You read them, act, and sign off without opening another tab.

Can I turn reminders off?

Yes. AI Plan Reminders are on by default, and you can switch them off in your settings.

Which EMRs does it work with?

AI Plan Reminders work wherever Medulla scribes: Med Access and Oscar-based EMRs.

Is patient data secure?

Yes. Everything is encrypted in transit and at rest, and Medulla is PIPEDA compliant, built and hosted in Canada. Read our privacy notice.
