Gynecologic Cytology and HPV Requisition (Non-OCSP)

HPV testing is publicly covered in Ontario only through the Ontario Cervical Screening Program, so a patient outside it, such as someone without OHIP or not due under the program, needs this private-pay form. The patient is invoiced by LifeLabs, which is why the form needs both your signature and theirs.

What the non-OCSP requisition is for

HPV testing has replaced cytology as the primary cervical screening test in Ontario, and it’s publicly covered through the Ontario Cervical Screening Program. LifeLabs says provincial coverage is available once every five years for eligible patients. This requisition is for patients outside the program, for example those without OHIP coverage or not eligible for program testing, who choose to pay. The form states it should only be used for patients outside the OCSP.

Patients in the program go on the Ontario Health OCSP requisitions, one for screening and one for colposcopy follow-up.

Who completes it

You complete the clinical sections and sign the acknowledgement that you’ve told the patient it’s a private-pay test. The patient signs their own acknowledgement that payment to LifeLabs is required. They don’t fill in any other part.

How to fill in the requisition

Requesting clinician and patient details

Enter your name, address, billing number and phone, any copy-to clinicians with all fields filled in, and the patient’s health card number and version, sex, date of birth, chart number, phone, address and names as on the health card.

Test requested

Choose one. Under screening: HPV test, which includes reflex cytology if HPV-positive, or cytology only. Under colposcopy: co-test with HPV and cytology, HPV only, or cytology only. Ordering both columns by mistake is easy, since the form lists five options side by side.

Specimen details and clinical status

Enter the collection date and the first day of the last menstrual period. Check the site, whether the cervix looks normal or abnormal, and the clinical status: post partum, pregnancy, postmenopausal, postmenopausal bleeding, hormone replacement therapy, IUD, or other. Record hysterectomy as subtotal, with cervix present, or total, with no cervix.

Clinical history and remarks

Explain abnormal findings, prior results, and anything checked as other. The form warns that inadequate clinical information may hinder diagnosis.

Payment acknowledgement

You sign that you told the patient this is a private-pay test and that they’ll be invoiced. The patient signs and dates their acknowledgement that payment is required.

Common reasons it’s delayed

  • Used for a patient who is eligible for the OCSP.
  • More than one test option checked.
  • Patient’s payment acknowledgement unsigned.
  • Last menstrual period or hysterectomy status left blank.
  • An abnormal cervix checked with no explanation in the remarks.

Submitting and who pays

The requisition travels with the sample to LifeLabs. The patient pays: LifeLabs mails them an invoice with payment instructions, at the price on the date of collection.

Questions

Questions we hear a lot.

How much does the patient pay?

The requisition lists $95 for the HPV test and $60 for cytology, charged at the price on the date of collection. LifeLabs mails the patient an invoice.

When should I use this instead of the OCSP requisition?

Only for patients outside the Ontario Cervical Screening Program. LifeLabs says provincial coverage is available once every five years for eligible patients, and eligible patients should be screened through the program on the OCSP requisition.

Can private insurance cover it?

Sometimes. LifeLabs encourages patients to check with their private health insurer.

Does the patient have to sign?

Yes. The patient signs to acknowledge that payment to LifeLabs is required, and you sign to confirm you told them it's a private-pay test.

Is patient information secure?

Yes. Medulla has a SOC 2 Type II report from an independent auditor, is HIPAA, PIPEDA, and PHIPA compliant, and is built and hosted in Canada, so patient data stays in the country and never gets pasted into consumer AI tools. Everything is encrypted in transit and at rest. See how we protect patient data.
