Form 7

Doctor Progress Report

After the first report, WCB Manitoba relies on progress reports to see whether the worker is recovering and when they can go back to work. Your answers on recovery, complications and restrictions shape whether benefits continue, whether WCB arranges further care, and how the employer plans a return.

What the Doctor Progress Report is for

The progress report updates WCB Manitoba on a claim it already knows about. WCB’s fee policy calls it Form 7, and the number appears in the form’s header. It’s shorter than the first report: it drops the employer and incident sections and adds questions about change, recovery and referral. Workers on opioids also have separate WCB opioid management reports.

Who completes it

The treating physician completes and signs the whole form. There’s no worker section.

How to fill in the report

Worker information and injury details

Enter the worker’s name, address, date of birth and PHIN, the claim number, the date of the incident and the area of injury. Use the same area of injury as the first report unless it has genuinely changed.

Examination findings and diagnosis

Give the examination date and ICD code, and say whether the diagnosis has changed. If it has, state the new diagnosis. Record subjective complaints and objective findings, including range of motion, muscle testing and neurological status, so WCB can compare them with earlier visits.

Recovery, tests and referrals

Answer whether recovery is satisfactory. If not, name the complications or other factors slowing progress, such as a pre-existing condition, a delay in treatment or psychosocial barriers. List any tests with location and date and attach results. If you’ve referred to a consultant, give their name, address and the appointment date.

Treatment plan

Describe the current treatment and give the date of the next visit.

Work capabilities

Give the date the worker can return to regular duties, or mark it unknown. If they can do alternate or modified work, list the restrictions in functional terms and their duration in weeks. Update restrictions at each visit, since WCB and the employer plan around the latest report.

Physician information

Print your name and contact details, sign and date.

Common reasons it’s delayed

  • Recovery marked unsatisfactory with no explanation of what is impeding it.
  • A changed diagnosis ticked with no new diagnosis written.
  • A referral noted with no consultant name or appointment date.
  • Restrictions copied from the first report without a new duration.
  • Missing claim number or signature.

Submitting and who pays

Fax the report to WCB Manitoba at 204-954-4999 in Winnipeg or 1-877-872-3804 toll free, with any test results attached. WCB pays the reporting fee under its agreements with Doctors Manitoba.

Questions

Questions we hear a lot.

When do I use the progress report instead of the first report?

Use the Doctor First Report after your first visit for the injury. Every later visit you report on the same claim uses the Doctor Progress Report.

Do I need the claim number?

The form has a claim number field at the top. By the time of a follow-up visit the worker usually has one, and including it lets WCB match the report to the claim.

Does WCB Manitoba pay for progress reports?

Yes. WCB pays a reporting fee for the Doctor Progress Report, at the rate set in its agreements with Doctors Manitoba.

Where do I send it?

Fax it to WCB Manitoba at 204-954-4999 in Winnipeg or 1-877-872-3804 toll free, as printed on the form.

Is patient information secure?

Yes. Medulla has a SOC 2 Type II report from an independent auditor, is HIPAA, PIPEDA, and PHIPA compliant, and is built and hosted in Canada, so patient data stays in the country and never gets pasted into consumer AI tools. Everything is encrypted in transit and at rest. See how we protect patient data.
