Form 8/11

Physician's Report

Form 8/11 is how WorkSafeBC learns what happened to an injured worker, what you found and when they can go back to work. The claim officer relies on your diagnosis and return-to-work answers, and the report is paid on a sliding scale that rewards sending it within days.

What Form 8/11 is for

Form 8/11 is one form used at two stages of a WorkSafeBC claim. Form 8, the Physician’s First Report, establishes the work injury claim, and only one is accepted per claim. Form 11, the Physician’s Progress Report, updates WorkSafeBC when something changes. Extra clinical detail goes on the Form 8/11 Addendum. Nurse practitioners and chiropractors have their own versions of the report.

Who completes it

The physician treating the worker completes the whole form, including the worker and employer details at the top. The worker doesn’t fill in a section.

How to fill in the report

Select F8 or F11

Tick one box only. Choose F8 for the first report when one of the criteria applies, or F11 when the worker’s condition or treatment has changed or they’re ready to return to work. For F11, you can skip the questions about first treatment and whether you’re the regular practitioner.

Worker, employer and injury details

Enter the date of service, the claim number if there is one, the worker’s name, birth date, address, phone and personal health number, and the employer’s name, phone and operating location. Give the date of injury or first treatment and who gave first treatment. State whether prior or other problems affect injury, recovery and disability, and whether the worker has been disabled from work.

Injury codes and clinical information

Write the diagnosis and add the CSA body part, nature of injury and ICD-9 codes. Under clinical information, describe what happened, the subjective symptoms, examination findings, investigations, treatment and medication, and any specialist consultation. Link the mechanism to the injury in plain terms.

Return-to-work planning

Answer whether the worker can do full duties full time. If not, list the current physical or psychological restrictions, stated as what the worker can and can’t do. Estimate the time before any return to the workplace, whether they’re ready for a rehabilitation program, the date of maximal medical recovery, and whether you’d like to consult a WorkSafeBC medical advisor.

Payee and practitioner

Complete the payee number and name and your practitioner number and name, so the report matches your invoice.

Common reasons it’s queried

  • Both F8 and F11 ticked, or a second Form 8 sent on the same claim.
  • A diagnosis with no mechanism linking it to the work activity.
  • “Not fit for full duties” with no restrictions listed.
  • Missing claim, practitioner or payee numbers, so the report can’t be matched.
  • A late report, which reduces or loses payment.

Submitting and who pays

Send it through Teleplan where you can. Teleplan reports pay at a higher rate than faxed ones. Paper reports go by fax to 604.233.9777 in the Lower Mainland or 1.888.922.8807 toll-free, or by mail to PO Box 4700 Stn Terminal, Vancouver. WorkSafeBC pays for the report, so the worker isn’t billed. Bill fee item 19927 when WorkSafeBC asks for a Form 8 that the criteria didn’t require, and 19900 for a Form 8 that should have been sent but wasn’t. Invoices should reach WorkSafeBC within 90 days of the service.

Questions

Questions we hear a lot.

When is a Form 8 required?

When you suspect the worker may be off work beyond the day of injury, when the claim is for a hernia, back condition, shoulder or knee strain or sprain, occupational disease or mental disorder, or when WorkSafeBC asks for one. Otherwise, your invoice for the visit is enough.

What's the deadline for Form 8?

WorkSafeBC asks for it within three business days of the first visit. Payment is on a sliding scale: Form 8 reports received within one business day and Form 11 reports within three get the highest rate, and nothing is paid after six business days.

When should I send a Form 11?

When recovery is slower than expected, the condition, treatment or return-to-work status changes, recovery runs past usual norms, more than four weeks have passed since the last Form 11, or WorkSafeBC requests one.

Is Form 8/11 the same as Ontario's WSIB Form 8?

No. WSIB Form 8 is Ontario's health professional's report to a different board. Form 8/11 is used only for WorkSafeBC claims in British Columbia.

Is patient information secure?

Yes. Medulla has a SOC 2 Type II report from an independent auditor, is HIPAA, PIPEDA, and PHIPA compliant, and is built and hosted in Canada, so patient data stays in the country and never gets pasted into consumer AI tools. Everything is encrypted in transit and at rest. See how we protect patient data.
